Pegpleranib + aflibercept is a combination therapy investigated for the treatment of neovascular age-related macular degeneration (nAMD). Pegpleranib is an aptamer that selectively binds to platelet-derived growth factor-BB (PDGF-BB), preventing it from interacting with its receptor PDGFR-β, thereby disrupting pericyte support of abnormal blood vessels and promoting regression of neovascular lesions. Aflibercept is a recombinant fusion protein that acts as a decoy receptor for vascular endothelial growth factor (VEGF)-A, VEGF-B, and placental growth factor (PlGF), thereby inhibiting pathological angiogenesis and vascular permeability. The combination is designed to target both VEGF-driven and PDGF-driven pathways involved in abnormal blood vessel growth in the retina[1][3][5].
