Pegylated interferon alpha + ribavirin is a combination antiviral therapy used primarily for the treatment of chronic hepatitis C virus (HCV) infection. The regimen consists of a pegylated form of interferon alpha (either alfa-2a or alfa-2b), which is a cytokine that exerts its effects by binding to cell surface receptors and inducing antiviral, antiproliferative, and immunomodulatory activities. Pegylation increases the half-life of interferon alpha, allowing for once-weekly dosing. Ribavirin is a nucleoside analog with broad-spectrum antiviral activity; it inhibits viral RNA synthesis and viral mRNA capping. Together, these agents act synergistically to enhance sustained virologic response rates in HCV-infected patients compared to monotherapy. This combination was considered standard-of-care before the advent of direct-acting antivirals[1][3][5].
