Drug intelligence / Profile preview
pelareorep + oxaliplatin + irinotecan + leucovorin + fluorouracil + atezolizumab is an investigational combination regimen consisting of the intravenously delivered oncolytic reovirus pelareorep, the modified FOLFIRINOX chemotherapy regimen (oxaliplatin, irinotecan, leucovorin, and fluorouracil), and the anti-PD-L1 monoclonal antibody atezolizumab. This combination is being evaluated as a first-line therapy for patients with newly diagnosed metastatic pancreatic ductal adenocarcinoma (PDAC) in Cohort 5 of the Phase 1/2 GOBLET trial. Pelareorep selectively infects and lyses cancer cells, releasing viral double-stranded RNA that stimulates a pro-inflammatory response, increases T-cell infiltration, and upregulates PD-L1 expression. This immunologically "hot" tumor microenvironment is intended to synergize with atezolizumab, which blocks PD-L1 to restore T-cell-mediated anti-tumor activity, while the modified FOLFIRINOX regimen provides cytotoxic tumor debulking.
