Overview

Combination therapy consisting of **pelareorep** (an intravenous oncolytic reovirus), **pembrolizumab** (a PD-1 immune checkpoint inhibitor monoclonal antibody), and **gemcitabine** (a nucleoside analog antimetabolite chemotherapy) has been investigated primarily for the treatment of advanced pancreatic ductal adenocarcinoma (PDAC) and other advanced solid tumors. Pelareorep induces lytic viral infection of tumor cells, leading to immunogenic tumor cell death and subsequent tumor microenvironment inflammation, upregulation of PD-L1 expression, and enhanced T-cell infiltration. Pembrolizumab blocks PD-1, reversing tumor-mediated T-cell inhibition. Gemcitabine provides cytotoxic anti-tumor activity via DNA chain termination. The combination aims to synergize viro-immunotherapy, checkpoint blockade, and cytotoxic effects, resulting in immunogenic modulation and anti-tumor immune activation. Clinical studies report that the regimen is generally well-tolerated, induces immune cell infiltration, increases T-cell clonality, and shows modest clinical benefit in refractory PDAC patients[1][3][5][6].
