This drug is a combination therapy consisting of pelareorep (an intravenous oncolytic reovirus, also known as REOLYSIN), pembrolizumab (a monoclonal antibody immune checkpoint inhibitor targeting PD-1, branded as Keytruda), and irinotecan (a topoisomerase I inhibitor chemotherapy, branded as Camptosar). Pelareorep induces a T cell-inflamed tumor microenvironment by infecting and lysing cancer cells and promoting antiviral and antitumor immune responses. Pembrolizumab blocks the interaction between PD-1 and its ligands, enhancing T-cell mediated antitumor immunity. Irinotecan inhibits topoisomerase I, preventing DNA replication and cell division in cancer cells. This combination has been investigated in phase 1b clinical trials for metastatic pancreatic adenocarcinoma patients who progressed after first-line treatment. It is designed to synergistically activate anti-tumor immunity and cytotoxic activity, and has demonstrated safety and early signals of efficacy in this population[1][2][4][5][6][7].
