PELF is a combination chemotherapy regimen consisting of cisplatin, epirubicin, leucovorin, and 5-fluorouracil. It was developed as a treatment for advanced gastric carcinoma. Clinical trials have shown PELF to be significantly more active than other regimens like FAMTX (5-fluorouracil, doxorubicin, and methotrexate) in terms of complete response rates and objective response rates. The regimen works through multiple mechanisms: cisplatin forms DNA crosslinks inhibiting replication, epirubicin intercalates with DNA and inhibits topoisomerase II, leucovorin enhances 5-FU activity, and 5-fluorouracil inhibits thymidylate synthase disrupting DNA synthesis.
