Overview

Pembrolizumab + carfilzomib + dexamethasone is an investigational combination therapy studied in relapsed or refractory multiple myeloma (rrMM). Pembrolizumab is a monoclonal antibody targeting the PD-1 receptor, thereby enhancing antitumor immune activity. Carfilzomib is a selective proteasome inhibitor, causing apoptosis of myeloma cells by blocking the protein degradation necessary for cell cycle progression. Dexamethasone is a synthetic corticosteroid with potent anti-inflammatory and immunosuppressive effects, used here to reduce immune-related adverse events and to potentiate antimyeloma activity. The combination has been evaluated in early-phase clinical trials (not approved for use) with preliminary efficacy in rrMM, but the clinical program was partially halted due to safety concerns about pembrolizumab-based regimens in this setting[1][5].
