Overview

pembrolizumab + CD19 CAR T (Shanghai Tong Ren Hospital) is a combination immunotherapy regimen being investigated for the treatment of relapsed or refractory B-cell malignancies, including B-cell acute lymphoblastic leukemia and B-cell lymphoma. The regimen consists of pembrolizumab (Keytruda), a humanized monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor to inhibit immune checkpoints, and autologous T cells engineered with a chimeric antigen receptor (CAR) targeting the CD19 antigen. The CAR construct utilized by Shanghai Tong Ren Hospital typically includes an anti-CD19 single-chain variable fragment (scFv) linked to CD3-zeta and CD28 costimulatory signaling domains. By combining these two agents, the therapy aims to leverage the direct tumor-targeting capabilities of CAR T cells while using pembrolizumab to mitigate PD-1-mediated immunosuppression and CAR T-cell exhaustion, potentially improving clinical outcomes in patients with aggressive B-cell cancers.
