Overview

Pembrolizumab + cyclophosphamide is a combination therapy used primarily in oncology clinical trials. Pembrolizumab is a humanized monoclonal antibody that targets programmed cell death protein 1 (PD-1), blocking its interaction with PD-L1 and PD-L2, thereby enhancing T-cell mediated immune responses against tumors[6]. Cyclophosphamide is an alkylating agent that interferes with DNA replication and suppresses the immune system, but at low (metronomic) doses can modulate the tumor microenvironment to enhance immunotherapy efficacy. This combination has been studied in advanced cancers such as melanoma and ovarian cancer, where it aims to improve anti-tumor activity by combining immune checkpoint inhibition with cytotoxic or immunomodulatory effects of chemotherapy[3][8].
