Pembrolizumab + disitamab vedotin is an investigational combination therapy being studied primarily for the treatment of HER2-expressing, locally advanced or metastatic urothelial carcinoma. Pembrolizumab is a monoclonal antibody that targets programmed cell death protein 1 (PD‑1), acting as an immune checkpoint inhibitor to enhance antitumor immune responses. Disitamab vedotin is an antibody-drug conjugate consisting of a fully humanized HER2-directed monoclonal antibody (disitamab) linked to the cytotoxic agent monomethyl auristatin E (MMAE) via a protease-cleavable linker. Disitamab vedotin exerts its effects through direct cytotoxicity, bystander effect, and immunogenic cell death in HER2-expressing tumor cells. The rationale for combining these agents is based on evidence that MMAE-based ADCs can induce immunogenic cell death and may synergize with PD‑1 inhibition to improve clinical outcomes[1][2][8].
