Overview

Pembrolizumab + docetaxel + prednisone is a combination therapy evaluated for metastatic castration-resistant prostate cancer (mCRPC) in chemotherapy-naïve patients who progressed after next-generation hormonal agents. - **Pembrolizumab** is a humanized monoclonal antibody that binds to programmed cell death protein 1 (PD-1), blocking its interaction with PD-L1 and PD-L2. This releases inhibition of immune responses, enabling T-cell–mediated antitumor activity. - **Docetaxel** is a cytotoxic chemotherapy agent of the taxane class that promotes microtubule assembly and inhibits microtubule disassembly, interfering with cell division. - **Prednisone** is a synthetic glucocorticoid corticosteroid, used to suppress inflammation and immune responses and as supportive care for docetaxel. This triplet combination was assessed to see if adding pembrolizumab to standard docetaxel + prednisone improves overall survival and radiographic progression-free survival compared to docetaxel + prednisone alone for mCRPC. It has demonstrated antitumor activity and a safety profile consistent with the individual components in clinical trials, but in the large phase 3 KEYNOTE-921 trial, it did not significantly improve efficacy outcomes compared to standard chemotherapy[1][2][3][4].
