Overview

This combination immunotherapy regimen, developed by the Icahn School of Medicine at Mount Sinai, integrates an 'in situ vaccine' (ISV) approach with the PD-1 checkpoint inhibitor pembrolizumab. The ISV component is designed to transform a single tumor site into an individualized vaccine factory through three coordinated steps: recruitment of dendritic cells using Flt3L (CDX-301), induction of immunogenic cell death and antigen release via low-dose radiation therapy, and activation of the recruited dendritic cells using the TLR3 agonist Poly-ICLC (Hiltonol). This localized immune priming is intended to generate a systemic anti-tumor T-cell response, which is then further enhanced and sustained by the intravenous administration of pembrolizumab, a monoclonal antibody that blocks the inhibitory PD-1/PD-L1 pathway. The regimen is currently being evaluated in Phase 1/2 clinical trials for patients with indolent non-Hodgkin's lymphoma, metastatic breast cancer, and head and neck squamous cell carcinoma.
