**Pembrolizumab + Hemopurifier** is an investigational combination therapy consisting of pembrolizumab, a programmed death receptor-1 (PD-1) blocking monoclonal antibody, and the Hemopurifier, a proprietary extracorporeal medical device from Aethlon Medical. Pembrolizumab enhances immune-mediated tumor cell killing by blocking the PD-1 receptor on T-lymphocytes, thus preventing tumor cells from evading immune surveillance. The Hemopurifier is a blood filtration device designed to remove immunosuppressive extracellular vesicles (EVs) and exosomes produced by tumors, which are believed to contribute to cancer progression and resistance to immune checkpoint inhibitors. The device functions as a plasma separator with a lectin-based affinity resin targeting glycan structures on EV and viral surfaces. The combination aims to improve response rates to pembrolizumab by reducing factors in circulation that promote immune escape. It is under clinical investigation for safety and feasibility in patients with advanced or metastatic solid tumors, including head and neck cancer[1][3][4][6][7].
