This combination therapy is being evaluated in the PELICAN Phase II clinical trial (DP-IC001) led by Imperial College London for the treatment of chemotherapy-naïve patients with advanced high-grade neuroendocrine tumors (HG-NETs). The regimen consists of an induction phase using the PD-1 inhibitor pembrolizumab in combination with carboplatin and etoposide chemotherapy, followed by a maintenance phase of pembrolizumab plus the multi-kinase inhibitor lenvatinib. Pembrolizumab is a humanized monoclonal antibody that blocks the interaction between PD-1 and its ligands (PD-L1 and PD-L2), thereby enhancing T-cell anti-tumor activity. Lenvatinib targets multiple receptor tyrosine kinases including VEGFR1-3, FGFR1-4, PDGFRα, KIT, and RET. Carboplatin and etoposide are standard cytotoxic agents that induce DNA damage and inhibit topoisomerase II, respectively, to provide synergistic anti-tumor effects.
