Overview

pembrolizumab + MK-0482 is an investigational combination therapy composed of two monoclonal antibodies: pembrolizumab, an approved humanized IgG4 monoclonal antibody that targets the programmed death-1 (PD-1) receptor on T cells, and MK-0482 (also known as rolistobart), a novel humanized IgG4 monoclonal antibody that targets immunoglobulin-like transcript 3 (ILT3, also referred to as LILRB4) on myeloid cells. Pembrolizumab acts as a checkpoint inhibitor to enhance anti-tumor T cell activity by blocking the PD-1/PD-L1 immune evasion pathway. MK-0482 inhibits ILT3, an inhibitory receptor associated with immune tolerance and immunosuppression in the tumor microenvironment, particularly through its actions on myeloid-derived suppressor cells and tolerogenic dendritic cells. The combination aims to relieve tumor-induced immunosuppression and augment anti-tumor immune responses. The combination is currently being evaluated for safety, tolerability, and clinical activity in early-phase clinical trials for various advanced solid tumors, including non-small cell lung cancer and metastatic pancreatic cancer[1][2][3][5][7][9].
