This combination therapy consists of pembrolizumab (a monoclonal antibody targeting PD-1), MK-0482 (a novel humanized IgG4 monoclonal antibody targeting immunoglobulin-like transcript 3, ILT3), carboplatin, and paclitaxel (both cytotoxic chemotherapeutic agents). Pembrolizumab acts as an immune checkpoint inhibitor, blocking PD-1 to enhance anti-tumor immune responses. MK-0482 is designed to inhibit ILT3, a receptor that suppresses immune activation in the tumor microenvironment, thereby potentially relieving immunosuppression and augmenting immune-mediated tumor killing. Carboplatin is a platinum agent that causes DNA crosslinking and cell death, and paclitaxel stabilizes microtubules, inhibiting cell division. This multi-agent regimen is being studied for its potential synergistic anti-tumor efficacy, including in previously untreated or advanced solid tumors such as metastatic pancreatic ductal adenocarcinoma (mPDAC)[2][1].
