Overview

Pembrolizumab + olaparib is a combination therapy under investigation for the treatment of various advanced cancers, including metastatic pancreatic cancer with BRCA1/2 mutations, metastatic castration-resistant prostate cancer (mCRPC), non-small cell lung cancer (NSCLC), and homologous recombination deficiency (HRD)-positive advanced ovarian cancer. Pembrolizumab is a monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor on T cells, blocking its interaction with PD-L1 and PD-L2 to enhance immune-mediated tumor cell killing. Olaparib is a small molecule inhibitor of poly(ADP-ribose) polymerase (PARP), an enzyme involved in DNA repair; by inhibiting PARP, it prevents tumor cells from repairing DNA damage, leading to cell death—especially in tumors with defects in homologous recombination repair such as those harboring BRCA mutations. The combination aims to exploit potential synergy between immune checkpoint inhibition and impaired DNA repair pathways[3][5][8].
