pembrolizumab + paclitaxel + radiation therapy is a combination therapeutic regimen being evaluated in a Phase II clinical trial (NCT04400331) led by the University of Colorado, Denver. This regimen is specifically designed for patients with PD-L1-positive metastatic or locally advanced unresectable triple-negative breast cancer (TNBC). It consists of localized radiation therapy to metastatic sites followed by systemic administration of the PD-1 inhibitor pembrolizumab and the microtubule-stabilizing chemotherapy agent paclitaxel (or its albumin-bound formulation, nab-paclitaxel). The rationale for this combination is to utilize radiation to prime the immune system and release tumor antigens, thereby potentially enhancing the anti-tumor activity of the checkpoint inhibitor pembrolizumab, while paclitaxel provides direct cytotoxic effects and further modulates the tumor microenvironment to favor an immune response.
