pembrolizumab + paricalcitol is an investigational combination therapy being evaluated for the treatment of resectable pancreatic cancer. Pembrolizumab is a humanized monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor, acting as a checkpoint inhibitor to enhance the body's immune response against tumor cells. Paricalcitol is a synthetic analog of calcitriol and a selective vitamin D receptor (VDR) agonist. In the context of pancreatic cancer, paricalcitol is used to potentially reprogram the tumor stroma, reducing fibrosis and improving the infiltration of immune cells and the delivery of co-administered agents. This combination is notably being studied by the M.D. Anderson Cancer Center in the neoadjuvant setting, often in conjunction with standard chemotherapy regimens like gemcitabine and nab-paclitaxel.
