Overview

Pembrolizumab + propranolol is a combination therapy being investigated for the treatment of unresectable stage III or IV melanoma. Pembrolizumab is a humanized monoclonal antibody that acts as a checkpoint inhibitor by binding to the programmed cell death-1 (PD-1) receptor, thereby preventing its interaction with PD-L1 and PD-L2 and enhancing T-cell mediated anti-tumor activity. Propranolol is a non-selective beta-adrenergic receptor antagonist. The combination, pioneered in clinical trials by Roswell Park Comprehensive Cancer Center, aims to counteract the immunosuppressive effects of stress-induced beta-adrenergic signaling within the tumor microenvironment. Research suggests that blocking the beta-adrenergic pathway can improve the efficacy of PD-1 inhibitors by reducing the immunosuppressive nature of the tumor site, potentially increasing the objective response rate in treatment-naive patients.
