Overview

Pembrolizumab + trastuzumab is a combination of two monoclonal antibodies used in oncology. Pembrolizumab is a humanized IgG4 monoclonal antibody that targets the programmed cell death protein 1 (PD‑1) receptor on T cells, blocking its interaction with PD-L1 and PD-L2 ligands, thereby enhancing anti-tumor immune responses. Trastuzumab is a recombinant humanized IgG1 monoclonal antibody that binds to the extracellular domain of the human epidermal growth factor receptor 2 (HER2), inhibiting downstream signaling pathways involved in cell proliferation and survival, and mediating antibody-dependent cellular cytotoxicity. This combination has been approved for use with chemotherapy as first-line treatment for adults with locally advanced unresectable or metastatic HER2-positive gastric or gastroesophageal junction adenocarcinoma whose tumors express PD-L1 (CPS ≥1)[3][9][4]. The regimen leverages both targeted therapy against HER2 and immune checkpoint inhibition.
