Overview

This combination therapy involves the immune checkpoint inhibitor pembrolizumab, the HER2-targeted monoclonal antibody trastuzumab, and a chemotherapy backbone (such as FLOT or fluoropyrimidine-platinum doublets). It is being investigated by Memorial Sloan Kettering Cancer Center (MSKCC) for the treatment of HER2-positive esophagogastric cancers, including esophageal cancer, gastric adenocarcinoma, and gastroesophageal junction (GEJ) adenocarcinoma. Trastuzumab blocks HER2 proteins that promote tumor growth, while pembrolizumab inhibits the PD-1/PD-L1 pathway to enhance T-cell anti-tumor activity. The chemotherapy component (including agents like oxaliplatin, 5-fluorouracil, and docetaxel) provides additional cytotoxic effects. These trials evaluate the combination in both metastatic and resectable (perioperative) settings to improve clinical outcomes in HER2-positive patients.
