Pemetrexed + (carboplatin or cisplatin) is a chemotherapy combination regimen used primarily for the treatment of non-small cell lung cancer (NSCLC) and pleural mesothelioma. Pemetrexed is an antifolate antineoplastic agent that inhibits several folate-dependent enzymes involved in the synthesis of DNA and RNA, thereby preventing cancer cell replication. Carboplatin and cisplatin are platinum-based chemotherapeutic agents that cause DNA crosslinking, leading to apoptosis in rapidly dividing cells such as cancer cells. This combination exploits different mechanisms to maximize cytotoxicity against tumor cells. The regimen is administered intravenously, typically every 21 days for several cycles, often with vitamin supplementation (folic acid and B12) to reduce toxicity[1][3][6][8]. The primary indications are advanced or metastatic non-squamous NSCLC and malignant pleural mesothelioma[1][2][5].
