Pemetrexed is a small molecule antifolate antineoplastic agent that inhibits folate-dependent enzymes involved in the synthesis of thymidine and purine nucleotides, thereby disrupting DNA and RNA synthesis essential for cancer cell replication. It is primarily used for the treatment of non-small cell lung cancer (NSCLC) and malignant pleural mesothelioma, often in combination with other chemotherapeutic agents[1][2][5][7]. PF-3512676 (also known as CpG 7909) is a synthetic immunomodulatory oligonucleotide that acts as an agonist of Toll-like receptor 9 (TLR9), stimulating immune responses against tumors[8]. The combination was investigated for advanced NSCLC but clinical development was discontinued after phase III trials showed no additional efficacy over standard chemotherapy alone[6].
