Overview

Penicillin + cefuroxime refers to the two antibiotic treatment arms evaluated in a randomized clinical trial (NCT00261742) for the treatment of perianal streptococcal dermatitis in children. Penicillin is a narrow-spectrum beta-lactam antibiotic, while cefuroxime is a second-generation cephalosporin. Both agents exert their antibacterial effects by binding to penicillin-binding proteins (PBPs), which are essential enzymes for bacterial cell wall synthesis. By binding to PBPs, these antibiotics inhibit the final transpeptidation step of peptidoglycan synthesis, leading to bacterial cell lysis. The trial, conducted by the University Children's Hospital Basel, compared a 10-day course of oral penicillin with a 7-day course of oral cefuroxime, finding that cefuroxime provided superior clinical and microbiological outcomes for perianal dermatitis caused by Group A beta-hemolytic Streptococcus (GABHS).
