Penicillin G benzathine + penicillin G procaine is a combination injectable suspension containing two long-acting forms of penicillin G: **benzathine** and **procaine** salts. This formulation is administered by deep intramuscular injection and is designed to provide both sustained and intermediate levels of penicillin G for effective antibacterial activity while reducing injection site discomfort compared to benzathine penicillin G alone[5][6]. The combination acts by inhibiting bacterial cell wall synthesis through binding to penicillin-binding proteins (PBPs), resulting in bacterial cell death. It is primarily indicated for treatment of moderately severe infections caused by susceptible strains of streptococci (such as Streptococcus pyogenes) and pneumococci (Streptococcus pneumoniae), including streptococcal pharyngitis, upper respiratory tract infections, and as prophylaxis against rheumatic fever. This formulation is **not approved for treatment of venereal diseases such as syphilis or gonorrhea**. Patients with a history of hypersensitivity to penicillins or to procaine should not receive this combination[5][6].
