This combination therapy regimen consists of penpulimab, anlotinib, and gemcitabine, and is being investigated as a first-line treatment for metastatic nasopharyngeal carcinoma (NPC). Penpulimab (AK105) is a humanized IgG1 monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor; it is engineered to eliminate Fc gamma receptor (FcγR) binding, which theoretically reduces immune-related adverse events by preventing the depletion of PD-1+ T cells. Anlotinib is a small-molecule multi-kinase inhibitor that targets vascular endothelial growth factor receptors (VEGFR1/2/3), fibroblast growth factor receptors (FGFR1-4), platelet-derived growth factor receptors (PDGFRα/β), and c-Kit, thereby inhibiting tumor angiogenesis and proliferation. Gemcitabine is a nucleoside metabolic inhibitor that disrupts DNA synthesis. This specific triplet combination is currently being evaluated in a Phase Ib clinical trial led by investigator Chen Xiaozhong at Zhejiang Cancer Hospital.
