Pentastarch (hydroxyethyl starch 200/0.5) is a synthetic colloid plasma volume expander derived from amylopectin. It is characterized by a mean molecular weight of 200,000 Daltons and a molar substitution of 0.5. The drug is administered intravenously to treat hypovolemia and hemorrhagic shock by increasing the colloid osmotic pressure of the blood, which facilitates the expansion of plasma volume. While effective for volume resuscitation, its use in critically ill patients, particularly those with severe sepsis, has been associated with an increased risk of renal failure and mortality, leading to significant clinical restrictions and the termination of several trials in these populations.
