Overview

Pentoxifylline + pentaglobin is a combination therapy used primarily as an adjunctive treatment in neonatal sepsis and other severe infections, especially in very low birth-weight preterm infants. Pentoxifylline is a methylxanthine derivative and non-specific phosphodiesterase inhibitor with anti-inflammatory, immunomodulatory, antioxidant, and hemorheological properties. It improves microcirculation by increasing erythrocyte flexibility, reducing blood viscosity, inhibiting neutrophil activation, suppressing platelet aggregation, and modulating cytokine production (notably decreasing pro-inflammatory cytokines such as TNF-α and IL-6)[3][6]. Pentaglobin is an IgM-enriched intravenous immunoglobulin preparation with antibacterial and immunomodulatory effects; it enhances complement activation and neutralizes bacterial toxins more effectively than standard IVIG due to its IgM content[8]. The combination has been studied for its potential to reduce inflammation markers (such as C-reactive protein), improve clinical outcomes in neonatal sepsis (including reduced mortality or hospital stay), though evidence for additive benefit over monotherapy remains limited[1][7].
