Pepinemab + trastuzumab is an investigational combination therapy consisting of two monoclonal antibodies. Pepinemab is a humanized IgG4 monoclonal antibody that targets semaphorin 4D (SEMA4D), modulating the tumor microenvironment to enhance anti-tumor immune responses. Trastuzumab is a well-established monoclonal antibody targeting the human epidermal growth factor receptor 2 (HER2), inhibiting HER2-mediated signaling and promoting immune-mediated cytotoxicity against HER2-positive cancer cells. The combination aims to leverage both direct anti-tumor effects and modulation of the immune response in patients with HER2-positive metastatic breast cancer. This regimen has been studied in conjunction with dendritic cell vaccines and adoptive T-cell therapy to further augment immunogenicity[1][3][5].
