perifosine + imatinib

**Perifosine + imatinib** is an oral combination of two small molecule agents: *perifosine*, an alkylphospholipid Akt inhibitor, and *imatinib*, a kinase inhibitor targeting BCR-ABL, KIT, and PDGFRA. This combination has been studied primarily in patients with gastrointestinal stromal tumor (GIST) who have shown resistance or progression on imatinib monotherapy. The rationale for the combination is that perifosine’s inhibition of the Akt pathway could overcome or delay acquired resistance to imatinib by inhibiting downstream survival signals in tumors with KIT mutations. Clinical studies have assessed the safety and efficacy of the combination, with the main reported toxicities being gastrointestinal, hepatic, and potential ocular (ulcerative keratitis) adverse effects.
