Overview

This is a combination investigational therapy composed of three parts: a **personalized neoantigen peptide vaccine**, **poly-ICLC** (a synthetic double-stranded RNA adjuvant), and an **anti-PD-1 checkpoint inhibitor**. - The **personalized neoantigen peptide vaccine** is designed and manufactured individually for each patient, based on sequencing tumor-specific mutations (neoantigens). Synthetic peptides encoding these predicted neoantigens are selected to induce robust, tumor-specific T cell immune responses. - **Poly-ICLC** (polyinosinic-polycytidylic acid stabilized with poly-L-lysine and carboxymethylcellulose) is an immune adjuvant acting primarily as a TLR3 agonist; it enhances antigen presentation and further stimulates immune system activation. - **Anti-PD-1 inhibitors** (e.g., nivolumab, pembrolizumab) are monoclonal antibodies that block the PD-1 checkpoint on T cells, preventing immune suppression by tumor-expressed PD-L1 and enabling a sustained anti-tumor immune response. Mechanistically, this combination aims to provoke strong, neoantigen-specific T cell responses and maintain their activity within the tumor microenvironment by blocking immunosuppressive signals. This approach is under study for a range of solid tumors, especially in refractory or high-risk diseases, but is **not yet approved** for routine clinical use. Current evidence shows early-phase clinical trials (primarily **PHASE1** and **PHASE2**) in several cancer types, including non-small cell lung cancer, glioblastoma, and other solid tumors[1][3][5][7].
