**Personalized neoantigen peptide vaccine + sargramostim** is a combination immunotherapy comprising a customized cancer vaccine made from synthetic peptides corresponding to mutation-derived neoantigens found in the patient's own tumor and *sargramostim* (recombinant human granulocyte-macrophage colony-stimulating factor, GM-CSF) as an immune adjuvant. The vaccine is designed using genomic sequencing and bioinformatics to identify patient-specific tumor mutations, from which peptides are synthesized and administered subcutaneously. Sargramostim boosts the immune response by promoting dendritic cell activation and antigen presentation. The primary mechanism is to induce robust, tumor-specific cytotoxic T-cell responses against cancer antigens, with the goal of stimulating the patient’s immune system to recognize and kill tumor cells. This combination is chiefly being studied for advanced solid tumors, often as part of multi-agent regimens with immune checkpoint inhibitors.
