**pertuzumab + trastuzumab + aromatase inhibitor** is a combination regimen used primarily as first-line therapy for patients with HER2-positive and hormone receptor–positive metastatic or locally advanced breast cancer. Pertuzumab and trastuzumab are both recombinant humanized monoclonal antibodies targeting distinct domains on the HER2 receptor, providing dual HER2 blockade. Aromatase inhibitors (such as anastrozole or letrozole) are oral small molecules that inhibit estrogen production, reducing the stimulation of hormone receptor-positive breast cancer cells. This combination exploits both anti-HER2 targeted therapy and anti-estrogen hormonal therapy, addressing resistance mechanisms due to cross-talk between HER2 and estrogen receptor pathways. The therapy has demonstrated improved progression-free survival compared to trastuzumab and aromatase inhibitor alone, particularly in patients not requiring induction chemotherapy.
