Overview

The combination of pertuzumab, trastuzumab, carboplatin, and docetaxel—commonly referred to as TCHP—is a neoadjuvant and adjuvant chemotherapy regimen used primarily for the treatment of HER2-positive early or locally advanced breast cancer at high risk of recurrence. Pertuzumab and trastuzumab are monoclonal antibodies that target different epitopes on the Human epidermal growth factor receptor 2 (HER2), providing dual blockade to inhibit HER2 signaling more effectively than either agent alone. Docetaxel is a taxane-class chemotherapy that inhibits microtubule depolymerization, disrupting cell division. Carboplatin is a platinum-based chemotherapeutic that induces DNA crosslinking and apoptosis in rapidly dividing cells. The synergistic effect of this regimen results in higher pathological complete response rates compared to regimens lacking dual anti-HER2 therapy or using anthracyclines[1][5][8]. This combination is associated with improved disease-free survival but also carries risks such as neutropenia and diarrhea[8]. The regimen has been evaluated in pivotal trials such as TRYPHAENA.
