Pertuzumab + trastuzumab + paclitaxel is a combination regimen used primarily for the treatment of HER2-positive metastatic or locally recurrent unresectable breast cancer. Pertuzumab and trastuzumab are monoclonal antibodies that target different domains of the Receptor tyrosine-protein kinase erbB-2 (HER2), leading to complementary inhibition of HER2 signaling pathways. Paclitaxel is a taxane-class chemotherapeutic agent that disrupts microtubule function, inhibiting cell division. The combination has demonstrated improved progression-free survival and overall survival in clinical trials and is included in major guidelines as an effective first-line therapy for HER2-positive metastatic breast cancer[3][4][5][6][8]. After initial cycles with all three drugs, maintenance therapy typically continues with pertuzumab and trastuzumab alone[1]. The regimen is indicated for patients who have not received prior anti-HER2 therapy or chemotherapy for their metastatic disease, especially those unsuitable for docetaxel[5]. Pertuzumab binds to domain II of HER2, blocking dimerization; trastuzumab binds to domain IV, inhibiting ligand-independent signaling; paclitaxel stabilizes microtubules preventing mitosis[6]. The combination has shown high rates of pathologic complete response in neoadjuvant settings as well[8].
