Overview

PETN + ISDN is a combination of two long-acting organic nitrate vasodilators—pentaerythritol tetranitrate (PETN) and isosorbide dinitrate (ISDN)—used primarily in the management of ischemic heart disease, including chronic stable angina pectoris. Both agents act as nitric oxide donors, leading to relaxation of vascular smooth muscle, vasodilation, and reduced myocardial oxygen demand. PETN has been shown to have a favorable profile compared to other nitrates due to its lower propensity for inducing tolerance and endothelial dysfunction; this effect may be related to upregulation of antioxidant enzymes such as heme oxygenase-1. In contrast, ISDN can induce tolerance with prolonged use. The combination may be used when enhanced anti-anginal efficacy or individualized patient response is desired[1][2][4].
