Pfs230D1-EPA + Matrix-M is an experimental transmission-blocking vaccine (TBV) designed to combat *Plasmodium falciparum* malaria. The vaccine consists of a recombinant protein representing Domain 1 of Pfs230, a highly conserved protein found on the surface of malaria gametes, which is chemically conjugated to a recombinant exoprotein A (EPA) from *Pseudomonas aeruginosa* that acts as a carrier protein to enhance immunogenicity. This conjugate is formulated with Novavax's Matrix-M adjuvant, a saponin-based technology that stimulates a robust and durable immune response. The mechanism of action is unique in that it does not primarily protect the vaccinated individual from clinical disease but rather induces antibodies that are ingested by the *Anopheles* mosquito during a blood meal. Once inside the mosquito midgut, these antibodies bind to the Pfs230 protein, preventing the parasite from undergoing fertilization and further development, thereby breaking the cycle of malaria transmission. The candidate is being developed by the National Institute of Allergy and Infectious Diseases (NIAID) in collaboration with Novavax.
