The combination of phenylpropanolamine hydrochloride and caffeine is a drug combination that has been studied and used in the past. This combination has significant pharmacokinetic interactions, with phenylpropanolamine shown to increase plasma caffeine levels substantially. Phenylpropanolamine is a sympathomimetic agent that was once commonly used as a decongestant and appetite suppressant in over-the-counter and prescription medications. It works by constricting blood vessels, which helps reduce nasal congestion. However, it has been withdrawn from many markets since around 2000 due to an increased risk of hemorrhagic stroke. Caffeine is a central nervous system stimulant that increases alertness and can enhance the effects of certain pain relievers. When combined with phenylpropanolamine, there are important interactions to note: 1. Studies have shown that phenylpropanolamine can significantly increase plasma caffeine concentrations. In one study, subjects receiving 400 mg caffeine plus 75 mg phenylpropanolamine had mean peak plasma caffeine concentrations of 8.0 ± 2.2 micrograms/ml, which was significantly greater than after caffeine alone (2.1 ± 0.3 micrograms/ml)[1]. 2. The combination has been associated with increased side effects, particularly cardiovascular effects. Greater increases in both systolic and diastolic blood pressures occurred after the combination than after either drug alone[1][2]. 3. Severe, life-threatening, and occasionally fatal hypertensive reactions have been reported after the combined use of these substances[1]. Due to safety concerns, particularly the risk of hemorrhagic stroke associated with phenylpropanolamine, the FDA recommended in 2000 that consumers not use products containing phenylpropanolamine. Most products containing this ingredient have since been reformulated or withdrawn from the market in many countries, including the United States, Canada, and Australia[2][7]. The combination was sometimes found in cold and flu medications that contained multiple ingredients to address various symptoms. Some formulations included additional components such as chlorpheniramine maleate and paracetamol/acetaminophen[6]. Given the safety concerns and regulatory actions, this specific combination is generally no longer available in many markets, though phenylpropanolamine may still be available in some countries under certain restrictions.
