Phesgo + adjuvant endocrine therapy is a combination treatment regimen being investigated in the Phase II ADEPT trial (NCT04622319) for the treatment of anatomic stage I, hormone receptor-positive (HR+), HER2-positive (HER2+) breast cancer. The regimen consists of Phesgo—a subcutaneous fixed-dose combination of the HER2-targeted monoclonal antibodies pertuzumab and trastuzumab with hyaluronidase-zzxf—administered every 21 days for one year, alongside approximately five years of adjuvant endocrine therapy (such as tamoxifen or aromatase inhibitors like letrozole, anastrozole, or exemestane). Trastuzumab and pertuzumab work synergistically to block HER2 signaling and induce antibody-dependent cellular cytotoxicity (ADCC), while the endocrine therapy component inhibits estrogen-driven proliferation of HR+ cancer cells. This chemotherapy-sparing approach is designed to maintain high efficacy while reducing the toxicity associated with standard-of-care chemotherapy in early-stage HER2+/HR+ breast cancer patients.
