Overview

This is a reduced-intensity conditioning (RIC) regimen developed by the Eastern Cooperative Oncology Group (ECOG) for patients with myelodysplastic syndrome (MDS) undergoing allogeneic hematopoietic stem cell transplantation. The regimen, evaluated in the Phase II trial E1902, combines extracorporeal photopheresis (ECP), pentostatin, and low-dose total body irradiation (TBI). ECP is an immunomodulatory therapy where a patient's white blood cells are treated with a photosensitizing agent (psoralen) and ultraviolet A light ex vivo before being reinfused, aiming to induce immune tolerance and reduce the risk of graft-versus-host disease (GVHD). Pentostatin, a purine analog and adenosine deaminase inhibitor, provides lymphodepletion, while TBI offers additional myelosuppression. This combination is designed to facilitate donor cell engraftment with lower toxicity than traditional myeloablative conditioning protocols.
