pioglitazone + clomiphene citrate is a combination therapy investigated for the treatment of infertility in women with polycystic ovary syndrome (PCOS). Pioglitazone is a thiazolidinedione that acts as a potent agonist for the peroxisome proliferator-activated receptor gamma (PPAR-gamma), which improves insulin sensitivity and reduces hyperinsulinemia, common factors in PCOS-related ovulatory dysfunction. Clomiphene citrate is a selective estrogen receptor modulator (SERM) that acts as an estrogen antagonist in the hypothalamus, blocking negative feedback and stimulating the release of gonadotropins (FSH and LH) to induce ovulation. The combination is intended to enhance ovarian stimulation and improve pregnancy rates, particularly in patients who may be resistant to clomiphene monotherapy due to underlying insulin resistance. This specific regimen has been evaluated in clinical trials sponsored by PAEC General Hospital, Islamabad.
