This investigational combination therapy consists of two small molecule agents: dapagliflozin, a sodium-glucose co-transporter-2 (SGLT2) inhibitor, and a low dose of pioglitazone, a thiazolidinedione. Developed by Hamad Medical Corporation, the combination is being evaluated in Phase 3 clinical trials for the treatment of heart failure with preserved ejection fraction (HFpEF) in patients with type 2 diabetes mellitus. Dapagliflozin works by inhibiting SGLT2 in the proximal renal tubules, reducing glucose reabsorption and promoting osmotic diuresis, which has been shown to reduce the risk of heart failure hospitalization. Pioglitazone acts as a peroxisome proliferator-activated receptor gamma (PPAR-γ) agonist, improving insulin sensitivity and metabolic function. The combination aims to provide synergistic therapeutic benefits by addressing both the hemodynamic and metabolic components of heart failure in diabetic populations.
