Overview

Pioglitazone + tretinoin is an investigational combination therapy consisting of two FDA-approved drugs—pioglitazone, a peroxisome proliferator-activated receptor gamma (PPARγ) agonist primarily used for type 2 diabetes, and tretinoin, a retinoid (all-trans retinoic acid) used in dermatology and oncology. The combination has been studied in clinical trials for amyotrophic lateral sclerosis (ALS), aiming to leverage the anti-inflammatory effects of pioglitazone on microglia and the differentiation-promoting properties of tretinoin. In preclinical models, pioglitazone reduced neuroinflammation markers such as cyclooxygenase-2 and inducible nitric oxide synthase. However, clinical studies have not demonstrated significant efficacy in slowing ALS progression[1][3][5]. Both agents act via nuclear receptors that regulate gene expression involved in inflammation, metabolism, cell differentiation, and survival.
