piperacillin-tazobactam + meropenem

Piperacillin-tazobactam and meropenem are broad-spectrum beta-lactam antibiotics used as standard-of-care empiric therapy for febrile neutropenia in pediatric oncology patients. Piperacillin is an extended-spectrum penicillin that, along with the carbapenem meropenem, inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Tazobactam is a beta-lactamase inhibitor that protects piperacillin from degradation by bacterial enzymes. The Centre Hospitalier Universitaire Vaudois (CHUV) is conducting the DAR-ARC trial (Phase 4) to evaluate a personalized dosing strategy for these antibiotics. This study focuses on children with augmented renal clearance (ARC), a condition where rapid drug elimination can lead to subtherapeutic antibiotic levels, and utilizes therapeutic drug monitoring (TDM) to optimize initial dosage adjustments.
