Overview

Pirfenidone + sintilimab is an experimental combination therapy being investigated for the treatment of colorectal cancer with peritoneal metastasis. This regimen combines pirfenidone, an antifibrotic small molecule typically used for idiopathic pulmonary fibrosis, with sintilimab, a recombinant humanized IgG4 monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor. In the context of peritoneal metastasis, pirfenidone is hypothesized to modulate the dense fibrotic tumor microenvironment (desmoplasia), potentially enhancing the penetration and efficacy of immunotherapy and concurrent chemotherapy. Sintilimab acts as a checkpoint inhibitor, restoring T-cell antitumor activity. This combination is being studied in clinical trials led by Guoxiang Cai, often in conjunction with standard neoadjuvant chemotherapy (CapeOX).
