The intervention consists of the selective serotonin reuptake inhibitor (SSRI) escitalopram administered alongside a placebo. In the context of the study, this arm served as the comparator to evaluate the efficacy of adding ramelteon to standard escitalopram treatment for patients with panic disorder and comorbid sleep initiation insomnia. Escitalopram is a small molecule antidepressant and anxiolytic that primarily targets the serotonin transporter (SERT), inhibiting the reuptake of serotonin into the presynaptic neuron and thereby increasing synaptic serotonin levels. In this trial, escitalopram was used to manage the underlying anxiety and panic symptoms, while the placebo replaced the investigational sleep aid to determine its incremental benefit on sleep quality.
