Led by investigator Matthew James, the study evaluates the efficacy of cilastatin in preventing acute kidney injury (AKI) in hospitalized patients receiving nephrotoxic medications. The placebo consists of an intravenous infusion of normal saline (0.9% sodium chloride solution), which is prepared to be identical in appearance to the active drug (cilastatin sodium reconstituted in saline). It is administered every 6 hours for up to 24 hours following the last exposure to a nephrotoxic agent, serving as the comparative baseline to assess the therapeutic benefit and safety profile of the repurposed cilastatin.
