Plasma-lyte 148 + dextrose is an intravenous crystalloid solution used for fluid resuscitation and maintenance therapy. It consists of Plasma-lyte 148, a balanced, isotonic electrolyte solution containing sodium, potassium, magnesium, chloride, acetate, and gluconate, supplemented with dextrose (glucose). The electrolyte composition is designed to closely resemble human plasma, with acetate and gluconate serving as bicarbonate precursors to provide a buffering effect against acidosis. The addition of dextrose provides a caloric source to prevent ketosis and hypoglycemia. This combination is particularly utilized in pediatric and acutely ill populations to maintain hemodynamic stability and electrolyte balance while minimizing the risk of iatrogenic hyponatremia.
