Platelet Rich Plasma (PRP) is an autologous biologic therapy derived from a patient's own blood, characterized by a platelet concentration significantly higher than baseline. In the Phase 4 clinical trial conducted by Brooke Army Medical Center (NCT05161468), PRP is being evaluated for the treatment of acromioclavicular (AC) joint dysfunction. The preparation involves centrifuging whole blood—often using specialized systems like the PEAK PRP System—to isolate the platelet-rich fraction. Once injected into the AC joint, platelets undergo degranulation, releasing a suite of growth factors including platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), and vascular endothelial growth factor (VEGF). These factors activate signaling pathways that promote mesenchymal stem cell recruitment, collagen synthesis, and angiogenesis, thereby facilitating tissue healing and reducing pain in the dysfunctional joint.
